RESUMO

BACKGROUND: Improper compliance with antibiotic prophylaxis (AP) in surgery is associated with an increased risk of surgical site infection (SSI), and impacts the efficiency of healthcare. OBJECTIVE: Evaluate the impact of an intervention in compliance with AP in selected surgical procedures and its effect on antibiotic consumption and cost. METHODS: A prospective interventional study was performed in a community hospital from January to December 2022. The baseline period was considered January-April 2022 and the intervention period May-December 2022. All patients who underwent cesarean section, appendectomies, hernia surgery, open reduction and internal fixation (ORIF), abdominoplasty, and cholecystectomy during the study period were selected. The intervention includes staff education, pharmacy interventions, monitoring the quality of prescriptions and feedback, and improved role of anesthesia staff, and department champions. RESULTS: The study involved 192 and 617 surgical procedures in the baseline and intervention periods respectively. The compliance with timing, selection, dose, and discontinuation achieved 100%, 99.2%, and 97.6% from baseline figures of 92.7%, 95.8%, and 81.3%, respectively. The antibiotic consumption was reduced by 55.1% during the intervention with a higher contribution of other antibiotics (94.1% reduction) in comparison with antibiotics as per policy (31.2% reduction). The cost was reduced by 47.2% (antibiotic as per policy 31.9%, other antibiotics 94.2%). CONCLUSION: The implemented strategy was effective in improving the quality of antibiotic prophylaxis with a significant impact in reducing antibiotic consumption and cost.

RESUMO

Cardiopulmonary exercise testing (CPET) provides a noninvasive and integrated assessment of the response of the respiratory, cardiovascular, and musculoskeletal systems to exercise. This information improves the diagnosis, risk stratification, and therapeutic management of several clinical conditions. Additionally, CPET is the gold standard test for cardiorespiratory fitness quantification and exercise prescription, both in patients with cardiopulmonary disease undergoing cardiac or pulmonary rehabilitation programs and in healthy individuals, such as high-level athletes. In this setting, the relevance of practical knowledge about this exam is useful and relevant to several medical specialties other than cardiology. However, despite its multiple established advantages, CPET remains underused. This article aims to increase awareness of the value of CPET in clinical practice and to inform clinicians about its main indications, applications, and basic interpretation.

The consumption of alcoholic drinks represents one of the main risk factors for the involvement in traffic accidents. The objective of this study was to analyze the general panorama of alcohol consumption by medical students and the implications for traffic accidents. SciELO, PubMed and Virtual Health Library databases were consulted, and ten full articles available between 2010 and 2022, in Portuguese and English, were included. The results obtained were that the consumption of alcoholic drinks by students ranged from 76.6% to 81.2%, and that alcohol drinking exposes drivers to involvement in 68% more traffic accidents than those who are not exposed to such factor. It was concluded that medical students, a young population, have a high rate of consumption of alcoholic beverages, and the reasons involved point to the level of pressure of the medical school, high workload, seniority in the program, academic parties, and living without the parents

RESUMO

AIM: To examine and map alcohol consumption in children and adolescents with chronic pain (CP). METHOD: A scoping review of international databases (CINAHL, WOS, PubMed, PsycINFO, Scopus, Embase, and MEDLINE) and grey literature was conducted between September 2022 and February 2023. Documents addressing the relationship between alcohol consumption and CP in children and adolescents, published in English and Spanish between 2012 and 2023, were included. Those papers with a population suffering from CP derived from neurodegenerative diseases, chronic infectious diseases or cognitive impairment were excluded. We assessed the level of evidence (LE) and the degree of recommendation (DR) of the studies included in accordance with the Scottish Intercollegiate Guidelines Network. Finally, 11 documents were considered out of the 479 reviewed. RESULTS: The relationship between substance consumption and CP in adolescents is a scarcely investigated topic. While there is a trend towards lower rates of alcohol consumption in youth with pain, there is evidence suggesting an association, particularly in cases of intense pain. The use of various substances, both legal and illegal, for pain management underscores the importance of comprehensively addressing this phenomenon in this population. CONCLUSIONS: Further research is needed to assess the relationship between alcohol consumption in children and adolescents with CP.

This meta-analysis was done to investigate the role of intrathecal midazolam in lower limb surgeries regarding prolongation of spinal block, postoperative pain control and associated side effects. The included studies reported onset and duration of sensory and motor block, time to first request analgesia, 24hours opioid consumption, postoperative pain control, and associated side effects following use of intrathecal midazolam for lower limb surgeries. This review was performed following the PRISMA guidelines and using the online databases, Medline, Science Direct, Google scholar and Cochrane library. We registered this review with the PROSPERO database (ID-CRD42022346361) in August 2022. A total of 10 randomised controlled trials were included in this meta-analysis. Our results showed patients receiving 1mg intrathecal midazolam showed significantly faster onset of sensory block (P=.001 [CI: −0.98, −0.31]). Duration of sensory and motor block were also significantly prolonged in intrathecal midazolam group (P<.00001 [CI: 18.08, 39.12], P=.002 [CI: 0.45, 2]). Intrathecal midazolam also increased the time to first request analgesia (P=.0003 [CI: 1.22, 4.14]). Pain scores at 4 and 12hours postoperatively were significantly lower in patients receiving intrathecal midazolam (P=.00001[CI: −1.20, −0.47] and P=0.05 [CI: −0.52, −0.01] respectively). In conclusion, the addition of intrathecal midazolam to local anesthetics in lower limb surgeries results in early onset of sensory and motor block. It also increases the duration of sensory and motor block. The time to first request analgesia is increased. VAS pain scores at 4 and 12hours postoperatively were also lower without any increased side effects.(AU)

RESUMO

ABSTRACT Background: Kidney failure reduces life expectancy by one-third compared with the general population, and cardiovascular complications and poor cardiorespiratory fitness (CRF) are the main causes. We aimed to evaluate the association between severely low CRF and all-cause mortality risk in HD patients. Methods: This observational prospective cohort study followed-up patients receiving HD from August 2015 until March 2022. Cardiorespiratory fitness was evaluated through the cardiopulmonary exercise test, and the peak oxygen uptake (VO2peak) value was used to determine severely low CRF (< 15 mL∙kg−1∙min−1). Cox regression and univariate Kaplan-Meier analysis were used to evaluate the association of severely low CRF with mortality risk and survival rate. Results: Forty-eight patients were followed-up for a median of 33.0 [14.3 - 49.3] months. A total of 26 patients had severely low CRF. During the follow-up period, 11 patients (22.92%) died from all causes. From these, eight (30.8%) had severely low CRF. Even so, severely low CRF was not associated with crude death rates for patients stratified by CRF levels (p = 0.189), neither in unadjusted (HR 2.18; CI 95% 0.58−8.23) nor in adjusted (HR 1.32; CI 95% 0.31−5.59) Cox proportional hazard models. As a continuous variable, VO2peak was not associated with mortality risk (HR 1.01; CI 95% 0.84−1.21). Univariate Kaplan-Meier analysis showed that patients with severely low CRF did not have significantly worse survival rates than those with mild-moderate CRF (p = 0.186). Conclusion: Our findings indicated that severely low CRF was not associated with all-cause mortality in patients on HD. Despite severely low CRF being prevalent, larger cohort studies are needed to establish strong conclusions on its association with all-cause mortality.

Food and Nutrition Security (SAN) is the realization of everyone's right to access food in sufficient quantities, quality and regularly, without compromising access to other needs. In view of the increase in food and nutrition insecurity, observed in recent years in Brazil and in the world, concomitant with the disruption of programs and public policies combined with the incentive to promote the right to Food and Nutritional Security (SAN) in Brazil (2019-2022) , and the pandemic context that aggravated this problem, we are currently experiencing alarming levels of Food and Nutrition Insecurity (INSAN), including the serious one that is characterized by hunger, which preferentially affects a certain social class and region in Brazil, North and Northeast. The present study analyzed the food and nutritional security profile of families residing in the rural community of Sítio Laguinha, around a wind farm in the municipality of Caetés, in Pernambuco. This study can be characterized as a mixed descriptive study with a quantitative and qualitative approach, in order to identify the prevalence of food (in)security and the associated factors. A semi-structured questionnaire was used to obtain information on socioeconomic conditions and food consumption in the rural community of Laguinha, as well as the Brazilian Food Insecurity Scale (EBIA) to assess the situation of Food and Nutritional Security. Among the factors associated with the community, the vulnerabilities to which this community is exposed stand out, such as the location in a rural area that is difficult to move around, around a wind farm that brings with it impacts and the pandemic context. The results obtained showed a high prevalence of food and nutritional insecurity (88.4%) at all levels, mild, moderate and severe. Among the factors that may be associated with and aggravate the INSAN situation, we can mention the impact of the wind farm and the pandemic, with all associated socioeconomic impacts. The results obtained in this work bring relevant data about the situation of an invisible community that needs assistance to achieve food and nutritional security, a right established in our constitution. In addition, these data can serve as a basis for the implementation of public policies aimed at the community

14 a máx.

Objective: to describe the characteristics of resilience and its association with the use of alcohol and tobacco among adolescents living in Eastern Tijuana, in the Northern border of Mexico. Method: a descriptive cross-sectional study on a population of 7,611 adolescents from 11 to 16 years old, attending school in the city of Tijuana, Baja California. The sample was estimated in 633 participants, selected through cluster sampling. Sociodemographic variables were collected, their history of use of tobacco and alcohol, as well as resilience (RS-14) (minimum 14 to maximum 98 scores; very low, low, normal, high or very high). Descriptive bivariate statistics were used, as well as logistical regression models adjusted by gender for the “use of alcohol of tobacco” variable. Results: the study included 710 adolescents (50.7% were female); 46% of them had used alcohol or tobacco at some point in their lives, and 27.1% during the past year (31.5% female; 23.3% male; p= 0.005); 29% showed high resilience and 25.5% showed very high resilience. The most frequently obtained high scores were for the statements: I am someone who can be trusted in emergencies (42.5%), I feel proud of my achievements (41.3%), and Life makes sense (33.1%). A negative effect of resilience was found on alcohol and tobacco use at some point in life (ORa= 0.984; CI95% [0.978-0.990]) and during the past year (ORa= 0.987; CI95% [0.981-0.994]). Conclusion: the higher the resilience, the lower the likelihood of using alcohol or tobacco at some point in life and during the last year. Future interventions should consider these results.(AU)

Introduction: nutritional epidemiology seeks to understand the relationship between food consumption and the occurrence of diseases. Duringthe COVID-19 pandemic, the Longitudinal Study of Adult Health (ELSA-Brasil) used a reduced Food Frequency Questionnaire (FFQ) to comparediet quality between cohort stages. Objective: to develop and validate an index created using a food quality score (FQS) using the FFQ of ELSA-Brasil.Methods: a cross-sectional study carried out between August 2020 and March 2021 with 4262 participants. For each frequency of consumptionof healthy and unhealthy food markers, a score was given. Validity and reliability were tested using Pearson’s correlation, principal componentanalysis (PCA) and Cronbach’s alpha. Tests were performed to verify differences between individual characteristics and FQS.Results: the PCA indicated a total variability of 42.5 %. Women and individuals with higher age, physical activity and per capita income had ahigher FQS, while those who reported higher consumption of alcohol and being smokers had a lower score. Cronbach’s alpha was 0.47.Conclusion: the FQS showed a satisfactory validity and reliability and was associated with socioeconomic and lifestyle data. Therefore, it is auseful tool for comparing dietary information, thus contributing to nutritional epidemiology.(AU)

Background: alcohol consumption is a public health problem that impacts the health, social and economic spheres. The objec-tives of this study were to describe the characteristics of alcohol-related emergencies (ari) in an area of high recreational tourism, and the implications of the covid-19 pandemic on this activity. Methods: a descriptive cross-sectional study of the period of ari emergency activity in the costa del sol hospital area during the years 2019-2021 was carried out. A stratified descriptive analysis was performed according to the covid-19 pandemic period, including the calculation of the incidence of ari emergencies attended daily. Descriptive analysis was performed evaluating differences between the three periods using the chi-square test for qualitative variables, and the kruskal-wallis test for quantitative variables. Results: during the study period, 479,204 hospital emergencies were recorded, of which 0.51% were identified as ari emergen-cies, with an average of 2.2 per day. This figure ranged from 2.7 emergencies per day during normality, 1 during confinement and 2.1 during new normality. The rate of ari emergencies for the period evaluated was 16.5 per 10,000 inhabitants/year. Conclusions: the patients treated for alcohol consumption in our series have a typical profile in terms of age (adult) and sex (male), although with a high relative weight of foreign patients. Restrictions due to confinement during the covid-19 pandemic have a positive impact on the emergency care of ari patients, although correlated with a generalised decrease in non-covid-19 related care activity.(AU)

RESUMO

The Mediterranean diet is the best evidence-based model for cardiovascular prevention. In addition to 2 major randomized secondary prevention trials (Lyon Heart and CORDIOPREV) and 1 primary prevention trial (PREDIMED) that have demonstrated these benefits, there is an unprecedented body of high-quality prospective epidemiological evidence supporting these beneficial effects. The key elements of this traditional pattern are the abundant use of extra-virgin olive oil and high consumption of foods of natural plant-based origin (fruits, vegetables, nuts, and legumes) and fish, along with a reduction in processed meats, red meats, and ultraprocessed products. Moderate consumption of wine, preferably red wine, with meals is an essential element of this traditional pattern. Although removing wine consumption from the Mediterranean diet has been associated with a reduction in its preventive efficacy, doubts have recently arisen about the possible adverse effect of even low or moderate intake of any alcoholic beverages. A new large Spanish trial, UNATI, which will begin in June 2024, will randomize 10 000 drinkers aged 50 to 75 years to abstention or moderate consumption. UNATI aims to answer these doubts with the best possible evidence.

Introduction: In Latin America, alcoholic beverages consumption is a main reason of problems related to physical, mental and social health. The National Commission for the Development of Indigenous Peoples states that indigenous populations are vulnerable problems related to alcohol consumption because a set of characteristics such as extreme poverty and educational backwardness. Our objective was to determine the effect of perceived stress on alcohol consumption in an indigenous population.Method: Study design was descriptive, correlational, and predictive. Results: Perceived stress was positively and significantly related to the number of drinks consumed in a typical day (rs = .211, p‹.01), it’s also related with alcohol consumption (AUDIT) (rs= .328, p ‹ .01), however, it was negatively and significantly related to age (rs = -.135, p ‹.05). The final Model presented a significant effect in the whole model (F (1,110) = 20.126, p = .001), explaining 39.3% of the variance of alcohol consumption. Perceived stress was found to have a positive and significant effect on alcohol consumption (B =.229, p ‹ .001).Conclusions: Perceived stress is a factor influencing excessive alcohol consumption in indigenous populations, drug use is considered a coping strategy to deal with stressful problems in daily life. The population presented problems with heavy episodic drinking, past year drinking with high prevalence and harmful drinking (AU)

RESUMO

INTRODUCTION: Frailty in the elderly is a concept in constant evolution, with a significant impact on the morbidity and mortality of patients. We assessed the associative strength of the Timed Up and Go test (TUG) and medication consumption in frailty and associated risk using various methods. MATERIAL AND METHODS: Observational, cross-sectional, multicenter study carried out in 128 Primary Care Centers distributed throughout Spain, has a total sample of 2422 patients over 65 years of age, estimating the prevalence of frailty with the TUG test. Descriptive analysis of the categorical variables and associative strength of TUG for frailty and medication consumption was performed with distribution of absolute and relative frequencies and multinomial logistic regression. SPSS 22.0 was used, considering statistical significance p=0.05. RESULTS: The prevalence of frailty in the sample was 13.7%, accepting as frailty those subjects who obtained a score >20s in performing the TUG test. A third of these subjects classified as frail took more than 30s to complete the test and 32.5% of those studied would be classified as "pre-frail". We found a slight difference in the prevalence of frailty if we analyze the data according to sex, being 10.8% in men compared to 15.8% in women. Regarding pharmacological consumption, 72.8% of the subjects consume 5 or more drugs daily with similar proportions by sex, and with an average consumption of 6.57 drugs. The drugs with the greatest association with the degree of frailty were drugs used for dementia (40% of individuals who consumed them were considered frail), antiparkinsonian drugs (38%), antipsychotics (34%), antianemics (26.2%), anticoagulants (22.2%) and antiepileptics (21.1%). CONCLUSIONS: The incidence of frailty in elderly patients in Spain is initially notable, being present in 13.7% through the application of the TUG test. The age variable is the one that is most significantly associated with the frailty of the elderly patient. An association was demonstrated between the degree of frailty and total medication consumption. The prevalence of frailty was 4.7% in the non-polypharmacy group compared to 15.4% in the polypharmacy group and reaching 23.4% in the extreme polypharmacy group. The drugs with a greater association with the patient's degree of frailty were those used in the neurological/psychiatric sphere (antidementia, antiparkinsonian, antipsychotic or antiepileptic drugs).

RESUMO

Abstract Repeatedly frying process of dietary edible oil has a potential role in the generation of free radicals. Therefore, questions have always been raised as to whether, there is an efficient and economical method to reduce the harmful effects of repeated use of frying edible oil. Since hibiscus has been stated to have a wide variety of therapeutic effects, it was important to investigate its properties against harmful effects of free radicals. The current study aspires to find out whether irradiated powder of hibiscus has a protective role against adverse effects of repeated use of frying edible oil. Thirty-five adult male albino rats were equally assigned into five groups. First group"G1" was fed with normal diet as control group, meanwhile, group"G2" the diet mixed with fresh oil, "G3" diet mixed with repeatedly frying oil only, "G4" diet mixed with frying oil treated with hibiscus and "G5" diet mixed with frying oil treated with irradiated hibiscus. Feeding duration was six weeks. Fatty acid analyses of oil as well as peroxide values were determined. Blood and liver samples were collected for biochemical analyses as well as histological study. Repeatedly heated cooked oil has significant increases in peroxide value, acid value, free fatty acid and both conjugated diene and triene compared with repeatedly frying oil treated with hibiscus. Also there are significant increases in cholesterol and triglyceride and impaired in liver functions in "G3"compared with others. In addition, relative to the hibiscus groups, there is a substantial reduction in oxygen consumption in "G3". Both hibiscus as well as irradiated hibiscus attract attention in order to play a vital and economical role against harmful effects of frequent use of frying edible oil on some biological functions but, irradiated hibiscus was more effective.

(AU)

Detailed knowledge of the epidemiology of alcohol consumption at the population level is essential to the design of effective public health programs. The objectives of this study were to estimate the prevalence of daily, sporadic, occasional, binge and hazardous alcohol consumption among the Galician population aged 16 years and over, and to characterize hazardous drinkers, taking into account the gender perspective. A cross-sectional study was conducted in 2017 among people aged over 15 years residing in Galicia in the framework of the Risk Behavior Information System (SICRI). In 2017, 7,841 adults were interviewed by stratified random sampling. Questions related to alcohol consumption in the last month were included and the prevalence of hazardous consumption was estimated using the AUDIT test. A multivariate logistic regression model was fitted to characterize hazardous drinkers. A total of 18.8% of the Galician population consumed alcohol daily and 3.4% had hazardous consumption. The prevalence of daily, occasional, sporadic, binge and hazardous drinking was higher among men and varied by age. The prevalence of daily-occasional and sporadic drinking was highest at middle age (45-64 years), and binge and hazardous drinking among those aged 16 to 24 years. The characteristics associated with hazardous drinking varied by gender, and are identified by common characteristics to both genders such as age or being a tobacco or cannabis smoker, and differential characteristics such as educational level. Typologies of consumption changed according to age and gender. Surveillance systems should regularly monitor alcohol consumption in order to adapt prevention measures to changes in drinking patterns.

(AU)

The co-occurrence of substance use disorders (SUD) and other mental disorders (OMD) is assumed to be high but is, in fact, unknown in Spain; and it is approached from different healthcare networks. The objective of the present study was to know the prevalence of this co-occurrence, both in specific addiction treatment networks and in mental health networks, in Spain. An observational, multicenter cross study, with a randomized sample, of patients under treatment for SUD or OMD in different Autonomous Communities of Spain was carried out (N=1,783). A specific ad hoc online questionnaire collecting sociodemographic variables, substance use and diagnoses of SUD and OMD was completed. The data obtained in the survey show a significant concurrence of SUD and OMD diagnoses (in more than 60% of the patients). A high prevalence of OMD was found in those patients receiving treatment for their SUD (71%), and also of diagnoses of any SUD (68.9%) and active substance use (50%, except tobacco) in people receiving treatment for diagnoses of OMD. Also were found significant relationships between addiction to certain substances and specific mental disorders: personality disorders with all SUDs; psychotic disorders with cannabis use disorder, but not cocaine use disorder; affective disorders with cocaine use disorder, and anxiety disorders with cannabis use disorder. This study provides preliminary information about the high coexistence in routine clinical practice of addictive disorders and other mental disorders in different treatment settings in Spain. (AU)

(AU)

Brain damage related to alcohol consumption is associated with impairments in cognitive functions, among which memory and verbal learning stand out. The main objective is to evaluate memory and verbal learning in a sample of 111 patients with alcohol use disorder (AUD) versus 78 with major depressive disorder (MDD) and 100 healthy controls. The evaluation included sociodemographic and clinical variables, the Hamilton Depression Scale (HDRS) and the California Verbal Learning Test (CVLT). One-way ANOVA was used for comparisons between the 3 groups and two-way ANCOVAS including different covariates. The one-way ANOVA shows that patients with AUD and MDD had scores similar to each other and lower than those of the control group (p <0.001), with the exception of the Cued CVLT (worse scores in MDD vs AUD, p <0.001). After including age, sex and years of completed studies as covariates, the differences between the AUD and MDD groups persisted compared to the control group (p ≤ 0.003) in all indices except for the Immediate Free CVLT and the Cued CVLT (worse performance in MDD vs AUD, p = 0.022 and p = 0.035, respectively). In the second ANCOVA, after controlling for depression severity, differences were only detected between AUD patients and healthy controls (p ≤ 0.007). Patients with AUD present a significant impairment in learning and verbal memory when compared with patients with MDD and with healthy people. (AU)

(AU)

Recent studies of transcranial direct current stimulation (tDCS), applied to the dorsolateral prefrontal cortex (CPFDL), have shown that they can reduce craving and smoking. However, there is little research that has evaluated the effects of tDCS on motivation and self-efficacy to quit smoking. The objective of this study was to evaluate the effects of tDCS on the pattern of consumption, motivation and perceived self-efficacy to quit smoking in 16 people with Tobacco Use Disorder (TUD). A time series design with intrasubject ABAB replication was used. The treatment consisted of applying 10 repeated sessions of tDCS at 1.5 mA for 20 minutes on the CPFDL (cathode F3 and anode F4), one daily session for two weeks (Monday to Friday). After a one-month rest phase, the intra-subject treatment was replicated under identical conditions. The entire intervention lasted nine weeks and was completed by 10 participants. The results showed a significant reduction in nicotine dependence, the number of cigarettes smoked and the levels of carbon monoxide (CO) in the exhaled air. Furthermore, we observed a significant improvement in motivation and perceived self-efficacy to quit smoking. These findings suggest that the tDCS, applied on the CPFDL, may be an effective technique to use as adjunctive therapy to other pharmacological and / or psychological strategies used in the Addictive Behavior Units, or other drug addiction care centers. However, more studies are needed to investigate the interaction between the effects of nicotine and tDCS to find the optimal treatment strategy. (AU)
